Medical Forms

¨ posted 02/01/09:  BSA has just released a new Annual Health and Medical Record form.  This form is to be used for youth and adults of all ages for monthly campout/outings, summer camp, and high-adventure activities such as Lenhok’sin.  The National BSA high-adventure camps Philmont, Sea Base, and Northern Tier will still have their own unique medical forms.

Those going to Hawk Mountain summer camp or Lenhok’sin high-adventure camp this year should use this new form.

PAST YEARS ARCHIVE:

Adults 40 and over – you need the Seabase Medical Form, if going to Seabase, and Medical Form Class 3 below for all other summer events.  These require a medical exam within 1 year of camp date.  Note: The Seabase Med Form can also be used for Lenhok’sin and Hawk Mountain.  Adults under 40, follow the rules below for youth.

Youth attending Seabase – You need the Seabase Medical Form below.  This requires a medical exam within 1 year of camp date. (This will also be valid for Lenhok’sin and Hawk Mountain merit badge summer camp.)

Youth attending Lenhok’sin – You need the Medical Form Class 3 below.  This requires a medical exam within 1 year of camp date. (Seabase Medical Form can also be used.)

Youth attending only Hawk Mountain merit badge summer camp – You can use the Medical Form Class 1 & 2 below (which requires a medical exam within 3 years of camp date AND a new parent update and a copy of the medical exam signed by a medical professional).  (Seabase Medical Form and the Medical Form Class 3 can also be used.)

 

¨ Medical Form Class 1 & 2 - For persons under age 40 attending regular summer camp and not engaging in High-Adventure activities.  Note:  If the Scoutmaster or you have a medical form from a previous year, for which a medical professional has signed based on a health examination within 3 years of camp date, then the parent only needs to complete and sign the first part of the new form (the Class 1 part, which is the first page and the top of the second page). Check with the Scoutmaster.

¨ Medical Form Class 3 - For persons age 40 and over attending regular summer camp or any person engaged in High-Adventure activities.

¨ Seabase Medical Form - Special form for those attending Seabase.
